People receiving the award are designated as “OMTA Fellows” and are recognized at the annual OMTA State Conference Banquet. For a $150.00 contribution to OMTA, a local association or individual may honor an OMTA member or a friend of OMTA. Donors are encouraged to purchase a banquet ticket for the OMTA Fellow Awardee. Deadline April 1st.

To designate an OMTA Fellow, you must submit the following:
- Name of Fellow being honored
- Name of person briefly introducing the Fellow
- Donation of $150 via credit card using the form below or check payable to “OMTA”, mailed to the OMTA Fellow Chair, Charlotte Dumesnil.
